Putting It All Together: Rush/Cook County Team Shows Value of Ancillary Data in Case Interpretation

After winning praise from around the world for its jam-packed Main Event, ASNC2020 returned on Sept. 30 with a Cases with the Aces session featuring faculty from Rush University Medical Center and Cook County Health. Together Rami Doukky, MD, FASNC, Rupa Mehta Sanghani, MD, FASNC, and Brian Clark, MD, presented nine cases, each including PET or SPECT images, clinical and ancillary data, as well as technical considerations.

"Putting it all together" was a common refrain as the presenters wrapped up each case and highlighted teaching points. Here are a few of the takeaways they emphasized:
- Which quality-control steps they take on every study and data set;
- Why they routinely view gated images in gray scale;
- How to calculate "steal phenomenon";
- Why any drop in ejection fraction seen on a PET scan is a red flag;
- Tips on what to do if you're uncertain about a wall-motion abnormality;
- Why radiology standardly over-reads their studies;
- What increased RV uptake reveals;
- Why it is crucial to "respect the ECG" and interpret images in the context of other data;
- When to explain discordant findings to referring physicians; and
- How to use LIST mode when troubleshooting cases with significant blood pool.
